Immunization Requirements
   

The Massachusetts Department of Public Health requires that all school children be properly immunized. All students who are new to Massachusetts schools must also comply with this requirement.
The only exceptions that can be made are for religious or medical reasons, and a signed statement by the parent/guardian must be on file in the health record. The School Nurse will notify parents/guardians of students with incomplete records. Immunizations must be completed prior to entry into school. Incomplete immunizations will result in exclusion from school until the requirements are met. The Massachusetts Department of Public Health has established the following requirements for school entry:

Measles Vaccine (MMR) All students entering seventh grade are required to have 2 doses of measles vaccine.
Diphtheria, Tetanus, Pertussis Vaccine (DPT) At entrance to the 7th grade, 1 booster dose of Td is required if it has been 5 years or more since the last dose.
Hepatitis B Vaccine Prior to entering the 7th grade, all students are required to have 3 doses of hepatitis B vaccine.

All students entering 7th grade are required to have one dose of varicella vaccine or physician certified reliable history of chicken pox disease.
